    • Referral Form: Fill out a referral form on the website to get started with the services you need.

  • Once you’ve submitted your referral form, our team will review the information and reach out to schedule a conversation about your personal goals and current needs. Together, we’ll begin the formal process of crafting a support plan tailored to you, ensuring a smooth and collaborative start to your journey with us.

  • We primarily specialize in mental health and psychosocial supports, but our services extend across a broad range of Core Supports. To explore the full scope of offerings, we encourage you to get in touch. We’ll work with you to identify which services align with your needs and desired outcomes.

  • We understand that life and circumstances can shift. At Prema Care, we’re committed to evolving alongside you adapting your services and making any necessary arrangements so you consistently receive the level of support you need, when you need it.

  • Depending on the type of support required, there may occasionally be a waitlist. However, we do our utmost to begin providing services as quickly as possible. We value your time and strive to accommodate new referrals promptly while maintaining the quality of care you deserve.

  • Yes. Although we’re currently awaiting final processing of our formal registration, Prema Care has already undergone the required auditing steps. In the meantime, we strictly follow the relevant standards and guidelines, undergoing regular audits of our policies, processes, and procedures, to ensure we consistently deliver high-quality, participant-focused support.

  • Switching is straightforward. Contact us to discuss your needs, and we’ll help you notify your current provider and transfer your services to Prema Care.

  • At Prema Care, we provide a transparent and flexible payment process tailored to suit your individual circumstances and funding options. Depending on your eligibility, services can be funded through various sources, including:

    1. NDIS (National Disability Insurance Scheme): If you are an NDIS participant, services are billed directly to your NDIS plan under the relevant funding categories.

    2. Medicare: Some services, such as counseling or allied health support, may be partially covered through Medicare rebates with a valid referral.

    3. WorkCover and TAC (Transport Accident Commission): For clients with work-related or transport accident injuries, services can often be funded through these schemes.

    4. Co-Payments: In cases where partial funding is provided (e.g., Medicare), co-payments may apply to cover the remaining cost.

    5. Self-Funded Options: Clients without access to other funding sources can choose to pay for services directly.
